Khon Kaen Highway District 3 (Ban Phai) was established in 1933. Later, in 1942, the district was dissolved and downgraded to a road maintenance division, then transferred to the Maha Sarakham Highway District. It remained under the Maha Sarakham Highway District until 1947, when it was re-elevated to a district, under the Khon Kaen Highway District. It was subsequently transferred to the Nakhon Ratchasima Highway District until 1994, when it was transferred back to the Highway Office 5 (Khon Kaen), where it remains to this day.
Khon Kaen Highway District 3 (Ban Phai) is a unit under the supervision of Highway Office 7 (Khon Kaen), responsible for the maintenance of roads. Responsible for road maintenance operations as follows:
- Concrete pavement: 76.472 kilometers
- Asphalt pavement: 451.192 kilometers
- Total actual distance: 434.114 kilometers
- Total distance per 2 lanes: 547.136 kilometers
